Florida law requires the campaign's official form, petition number, ballot summary, full amendment text, and financial impact statement before signatures can be collected. We will post that exact form here as soon as the Division of Elections publishes it.
Your print-and-mail checklist
When the official form is released, follow these four steps.

Download the official PDF
Use only the campaign form linked from this page or the Florida Division of Elections. Do not alter, crop, or reformat it.

Print and complete every required field
Use dark ink. Enter your name, legal residence, county, requested identification information, and the date. Incomplete forms cannot be validated.

Sign exactly as you are registered
You must be a registered Florida voter. Sign only once. The signature and date must be handwritten on the official form.

Mail it to your county elections office
Florida supervisors of elections verify initiative petitions by county. Use the official directory to find your county's mailing address before sending the form.
